• If You Are a Healthcare Professional:

• Support the delivery of the Amgen®️ Support Program (Program) to your enrolled patients.
• Provide your professional information (e.g., name and contact details) to Amgen for various program-related and administrative purposes such as direct communications to you, offering feedback on patient care and understanding your engagement within the program.

• If You Are a Patient:

• Enrol you in the Program and deliver the services and materials you request.
• Contact you for follow-up purposes related to the Program.
• Aprove the Program by using your information in de-identified, aggregated form. Aggregated data does not contain any personally identifying details.

• Aggregated Patient Information: Atlantis may share de-identified, aggregated patient data with Amgen to help improve the Program, understand trends (e.g., medication adherence or outcomes), and conduct research. No identifying patient information will ever be shared with Amgen.
• Healthcare Professional Information: Amgen may receive your professional details from Atlantis if consent has been provided to support the program management, understand the program performance and enable Amgen to communicate with you regarding the program where appropriate to improve the Program and providing relevant support materials.
• Third-Party Service Providers: Amgen and Atlantis may work with trusted third parties (e.g., IT providers, mailhouses) to help deliver the Program. These third parties are required to protect your personal information and only use it for program-related services.
• Program Continuation If Atlantis ceases to be involved in the Program, Atlantis may transfer your personal information to Amgen or a third party nominated by Amgen for purposes of continuing the program. Amgen will handle all personal information it collects from third parties about you for the purposes and in the manner described in this privacy statement.
• Adverse Event Reporting: To comply with regulatory requirements, Amgen must monitor the safety of its products. Reports of adverse events (e.g., side effects) or product quality issues may be shared with Amgen’s Pharmacovigilance team, regulatory authorities, or service providers. Amgen may contact you or your healthcare professional for further information if needed.
• Data Transfers: Personal information may be transferred outside Australia, such as to Amgen’s global offices or data centres. Any transfers comply with privacy laws, including the use of safeguards like Standard Contractual Clauses.

• Contacting you via email, SMS, or phone for program-related purposes.
• Collecting, using, and disclosing your information as described in this policy. You can withdraw your consent or opt out at any time by contacting privacy@atlantishealth.com

• Access, correct, or request deletion of your personal information.
• Request a copy of your data in a portable format.
• File a complaint with the Office of the Australian Information Commissioner if needed.
